Information you submit through general Site forms (before establishing a provider-patient relationship) is treated confidentially but is not, standing alone, considered PHI under HIPAA.

Telehealth visits are conducted using a secure, HIPAA-compliant video platform. By participating, you acknowledge:

• The telehealth platform may collect connection metadata (e.g., session length, IP address, device information) to deliver and secure the service
• Audio and video of your visit are not recorded by MyWellnessMD unless you provide advance written consent
• You are responsible for the privacy of your own environment during the visit
• Telehealth carries inherent technology risks (dropped connections, interception) that we mitigate but cannot fully eliminate
• Telehealth services are limited to patients physically located in California at the time of the visit, consistent with California medical licensure requirements

7. Third-Party Services

Our Site may use third-party services including but not limited to:

• Analytics (e.g., Google Analytics) to understand how visitors use the Site
• Appointment scheduling and patient portal platforms
• Website hosting and security providers
• Email and communication tools

Authorized agents may submit requests on your behalf with proof of authorization.

California “Shine the Light” Law

California residents may request information about the categories of personal information we disclose to third parties for their direct marketing purposes. We do not disclose personal information to third parties for their direct marketing purposes.

We retain your information for as long as necessary to provide services, comply with legal and regulatory obligations (including California medical record retention requirements — generally at least seven years for adult patients and longer for minors), resolve disputes, and enforce our agreements. Website analytics data is typically retained for a shorter period.
