Dr. Khan sees patient virtually, through a secure, HIPAA-compliant telehealth platform, so you can meet from the comfort of your own home or at the office in Santa Monica. We are out-of-network with all medical insurance plans, which means we choose not to work within insurance limitations. This allows Dr. Khan to dedicate your care to YOUR needs, not your insurance plan’s rules and time.
Even though we do not bill insurance directly, this does not mean that your insurance will not pay for our services — all of our services are billable. Your insurance will still cover lab work, imaging and other diagnostic testing, procedures, and prescriptions according to the terms of your health plan.

1. You pay out-of-pocket for your visit.
Payment in full is due at the time of service. We accept all major credit and debit cards, as well as HSA and FSA cards issued through Visa, Mastercard, American Express, or Discover.
2. We give you an itemized receipt (superbill) after each appointment.
Your insurance company requires a superbill to process your out-of-network claim. The superbill we provide is an itemized receipt listing the diagnostic (ICD) and treatment (CPT/HCPCS) codes for the care you received.
No, I do not accept patients outside of California.

All of our out-of-network services are billable to your medical insurance company. They will typically cover lab work, imaging and other diagnostic testing, procedures, and prescriptions according to the terms of your health plan.
NOTE: Reimbursement varies by plan and is never guaranteed. Call the member services number on your card and ask about your out-of-network deductible and reimbursement rate to know what to expect. Medicare does not reimburse out-of-network care, so you cannot submit superbills to Medicare.
What do I send to my insurance company?
You submit the superbill to your insurance company. They read the diagnosis and service codes to check if those services are covered by your plan, and apply the cost to your deductible. Insurance will send a reimbursement check directly to you.
